PATH is seeking candidates passionate about helping others make a positive change in their lives to join our CAL Optima Program as the Program Associate.
ABOUT OUR PROGRAMPATH's Orange County Health Programs services eligible Medi-Cal beneficiaries experiencing homelessness, including chronic homelessness, with the goals of improving health outcomes and/or decreasing utilization of high-cost services and increasing permanent housing placements.
WHAT IS A PROGRAM ASSOCIATE?As part of the CAL Optima Program, the program associate provides general administrative support to program leadership and case management staff. Position Responsibilities: -

• 
  
• Maintain and monitor data systems for the program and additional programs as needed.
  
• Tracks program enrollment /reconciles billing data and works cross functionally to ensure accuracy -
  
• Assists with new hire staffing in collaboration with the Human Resources department
  
• Administer client databases including generating reports, user management, and adherence to Homeless Management Information System (HMIS) policies and procedures
  
• Participate in Quality Assurance activities, designated or required programs and staff meetings, including internal and external training
  
  WHAT YOU BRINGWe're looking for candidates that:
  
  • 
    
  • Be highly proficient with Microsoft Office Software, specifically possess an advanced skillset using Excel (understand formulas, formatting, data manipulation).
    
  • Ability to coordinate multiple projects in a high-pressure and time-sensitive environment. -
    
  • Are highly organized and display excellent time management skills
    
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
    
  • Demonstrated knowledge of and/or experience with advanced case management techniques including critical time intervention, trauma informed care, harm reduction strategies, crisis intervention techniques and motivational interviewing. -
    
    PREFERRED QUALIFICATIONSAn excellent candidate for this role will: -
    
    • 
      
    • A bachelor's degree in a related field
      
    • Two (2) years of experience working in a relevant field.
      
    • Permanent supportive housing experience
      
    • HMIS experience
      
      MINIMUM QUALIFICATIONS To be considered for this position, we require all candidates:
      
      • 
        
      • One (1) year of experience in homeless services or related field and/or have an equivalent combination of education and experience. -
        
        MANDATORY REQUIREMENTSRegardless of qualifications, candidates must:
        
        • 
          
        • Have employment eligibility verification
          
        • Have or be able and willing to obtain CPR/First Aid training
          
        • Provide proof of full COVID-19 vaccination
