As a Telephonic HEDIS Outreach Coordinator, you will facilitate provider and member outreach to ensure member receipt of important clinical services to improve health outcomes. You will provide support for Healthcare Effectiveness Data and Information Set (HEDIS) quality initiatives and regulatory/contractual requirements including outreach to members who are noncompliant in the designated outreach services.

This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of (8:00am - 5:00pm). It may be necessary, given the business need, to work occasional overtime.--- -

If you are located in the state of Michigan, you will have the flexibility to telecommute (work from home) as you take on some tough challenges. -

Primary Responsibilities:

  • Call members to remind them of needed preventative screenings. The goal of these messages are to remind patients, identify if any screenings have already been performed and, if they have not been completed, to assist the beneficiary in any way possible to complete the screenings
  • Contact members to perform health screenings such as Health Risk Assessments and/or Social Determinants of Health (SDoH)
  • Interface with physicians, vendors, members, and internal staff telephonically or in person to obtain data

    Required Qualifications:

    • 2+ years of experience speaking with medical providers and members -
    • 2+ years of experience working with HEDIS gaps in care -
    • 2+ years of Member outreach and/or health education experience -
    • Ability to travel locally up to 5% of the time---for meetings -
    • Able to comply with UHG Telecommuting Policy and have a designated workspace inside the home with access to high-speed internet availability -
    • Reside in the state of Michigan -
    • Available to work Monday through Friday, 8 AM to 5 PM -
    • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.       Preferred Qualifications:

      • Bachelor's Degree (or higher) in a health-related field -
      • Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant -
      • CHW Accreditation -
      • Field based experience -
      • Care management experience -
      • Experience working in Managed Care -
      • Knowledge and/or experience with behavioral health or substance use disorders -
      • Knowledge of Medicaid and/or Medicare population -
      • Knowledge of HEDIS and NCQA -
      • Fluency in Spanish/Arabic -
