seniors. The PACE model is centered on the belief that it is better
for the well-being of seniors with chronic care needs and their
families to be served in their community whenever possible.
Seniors, their family members, and caregivers face many issues,
including transportation to appointments, management of
medications, coordination of medical care from different
specialists, lack of social interaction, and the ability to stay
alone at home. On Lok PACE participants receive in-home care
services and transportation to an On Lok PACE center for primary
medical care, social and recreational activities, and other senior

Lok as a PACE Center Director and lead a dynamic team dedicated to
transforming elder care! Your leadership will drive the delivery of
exceptional care to our participants, as you coach, lead, and
develop a team of compassionate healthcare professionals. In this
dynamic role, you will act as a liaison between On Lok's
administration, management team, and program staff, facilitating
seamless communication and collaboration across all disciplines.
You will oversee daily operations, quality assurance, and
compliance, making sure our center functions smoothly and
efficiently. As a PACE Center Director, you will play a pivotal

with our population. DUTIES / RESPONSIBILITIES: Program Leadership,
Staffing Management & Development Provides leadership and is
actively involved in the development and facilitation of
participant care planning, care coordination, risk management, and
service utilization. This includes program oversight and ensures
execution of all regulatory and compliance requirements.
Responsible for planning, organizing, and directing the day-to-day
operations of the PACE center. Maintains employee morale and uses
ongoing training/mentoring sessions to improve quality and
productivity and lessen employee turnover. Sets and reinforces
standards for acceptable levels of performance and behaviors.
Ensures quality, compassionate, and competent service delivery
across all On Lok external partnerships. Implement and strengthen
systems, workflows, policies, programs, and performance.
Demonstrates leadership by creating key partnerships across On
Lok's PACE centers, alternative care settings (ACS'), and others.
Serves as an effective link with On Lok's internal partnership,
administration, On Lok Senior Health Services management team, and
program staff through key indicator management, professional and
polished communication, productive collaboration, and facilitation
of change opportunities. Ensures continuity and consistency in
delivery and quality of services by ensuring the center is
adequately staffed. In collaboration with other leadership,
implements center staffing plan, supervises, and evaluates staff
performance, and promotes staff development. Ensures that all
interdisciplinary team (IDT) members are effectively and actively
participating in participant care planning, coordination, and
problem-solving, by providing ongoing coaching and training.
Monitor staffing trends including performance, turnover, overtime,
and employee recognition. Quality Assurance, Improvement &
Compliance Collaborates with the Quality Assurance, Practice
Management, Medical, and program leadership to continuously seek
ways to improve workflow efficiency and participant care process
efficiency resulting in an improvement of participant care
outcomes, and staff utilization. Stand up on-site compliance
monitoring and evaluation plan to continuously pursue audit
preparedness. Develops and implements corrective plans of action
for identified areas of improvement. Participant Care Management
Leads the coordination of the interdisciplinary team (IDT) and
center resources to develop, coordinate, monitor, and execute
participant care plan goals for all center participants. Ensures
staff are provided with the resources, training, and tools needed
to deliver all care planned goals. Identifies opportunities for
care plan innovation with the identification of new service
delivery options or modalities separating On Lok PACE from other
health plans and providers. Leads process improvement initiatives
and training with the interdisciplinary team to improve
collaboration between departments in the development of care plan
goals and execution. Ensures and reinforces oversight of excellent
IDT service delivery. Partners with the other PACE Center Directors
to design, standardize, and implement process improvement and
solutions to advance the daily operations. Strengthens
cross-functional partnerships with support departments to ensure
the promotion and utilization of advanced technology to capture
state-of-the-art processes and procedures for efficient use of
staff and communication with participants, customers, vendors, and
partners. Monitors compliance and adherence to regulator standards
on care planning. Manages against over-provision of resources
through effective preventative care planning for all participants.
Financials & Asset Management Effective management of resources and
systems to ensure the center meets its operational, human resource,
and budgetary goals. Provides oversight, management, and strategic
direction of center finances to ensure capital expenditures,
operational expenses, staffing, and participant care utilization
are within acceptable budgeted variance. Oversees facility assets,
space, equipment maintenance, and safety. Strategic Planning &
Project Execution Formulates and advances the goals and objectives
of the organization by developing specific performance goals,
implementing quality improvement plans with center staff, and
participating in establishing and monitoring financial projections.
Maintains active involvement in the promotion of On Lok in the
community. Partner with Marketing, Outreach, and PACEPartners to
strategize and develop ideas for greater outreach/visibility
efforts. Leads and designs standardization and continuous process
improvement efforts; uses change management methodologies and
creates excitement around change and improvement. Utilizes process
improvement techniques (i.e., Lean Six Sigma) and Project
Management tools as appropriate. Other duties as assigned.
QUALIFICATIONS (knowledge, skills, abilities): Bachelor's Degree in
Healthcare or related. 4-7 years of experience working with the
geriatric or frail population. 2-4 years experience in
healthcare-related operations and administration. 2-4 years of
financial management. Demonstrated experience and success in
leadership/supervisory positions. Proven ability as a skilled
facilitator with excellent communication and interpersonal skills.
Demonstrated ability to bring a broad perspective on health and
social issues and to bridge these issues in daily operations.
Documented success in formulating leading, and executing business
goals and objectives. Demonstrated competency in the use of
standard business software to manage business intelligence,
electronic health records system (EHR), enterprise business systems
(EBS), and Microsoft Suite. The above statements are intended to

compensation offer will be determined based on factors such as

range is expressed as an hourly rate regardless of FLSA status.
Exempt positions are paid salaried and are not subject to overtime.
