Licensed Vocational NursePOSITION SUMMARY:You will be responsible for providing nursing care to assigned patients under the direct supervision of a registered nurse (RN). General responsibilities may include giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documentation on patient progress.RESPONSIBILITIES:Provide direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of careObserve and report on the home environment and make recommendation that will enhance patient care, promote safety and prevent adverse effectsWork with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomesTake verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriatelyInform Health Care Providers of changes in patients condition in accordance with providers orders and documents communications accordinglyProvide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventionsProvide instructions to patient/CGs and staff in a timely mannerQUALIFICATIONS:Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse, and CPR certifiedAt least one year clinical practical nursing experience. *Home Health experience preferredMust have a vehicle, valid current drivers license, and automobile insuranceAbility to make home visits and provide skilled nursing assessments and care in the homeBENEFITS (available to full-time employees):Excellent pay, including 23 days of PTO per yearHealth, dental, vision, 401k, life insurance, short-term and long-term disabilityContinuing education & tuition assistanceGenerous referral bonus programFlexible schedulingCompany Vehicle Program with a paid gas card for eligible full-time employeesABOUT US:Our unique focus is on patient-centered care and evidence-based medicine.
