About the Organization and the Job:Heart of Hope Asian American
Hospice Care is a non-profit startup Hospice licensed by California
state since 2016. We are looking for a per-diem Home Health Aide
(HHA) to join our growing team in San Jose, California. It offers a
unique opportunity to work with a team that is passionate in
providing end-of-life care to the under-served community in the
Silicon Valley Area.High-level Job DescriptionUnder the supervision
of the registered nurse, the HHA provides personal care and related
non-professional household services necessary to meet the hygiene,
ADL, and comfort needs of the homebound patient in alignment. The
HHA receives functional guidance and supervision from the
interdisciplinary team. The HHA will be assigned in a manner that
promotes quality, continuity, and safety of a patient's care.Pay
Rate and BenefitsPay rate depends on relevant qualification and
experience.Hourly rate: $22-$27Visit rate: $50-$55We offer
part-time employee benefits including Paid Sick Leave; 401K

matching.Qualifications/RequirementsRequired Licensure and
CertificationMust be a Certified Home Health Aide in the State of
CaliforniaBasic Life Support for Healthcare Providers (BLS) CPR
certificationAutomobile insuranceDriver License, valid and in the
State of CaliforniaCompliant with Covid-19 healthcare personnel
requirement in all City, County, State and Federal levelExperience
and SkillsMinimum one year experience performing duties as an aide
in a home care setting or other health care related facility
requiredDirect patient care experience within the past three years
requiredWorking knowledge of medical terminology and health care
practices is required as well as understanding of universal
precautionsAbility to read and follow written instructions and
document care givenTolerance for body secretions, strong odors,
potential exposure to disease, and varied environmental
conditionsAble to cope with emotional stress tolerant of different
lifestyles, cultures, beliefs, and valuesKnowledge of and
commitment to hospice philosophy of careBilingual Chinese/English
