Description Currently hiring for a Home Health Registered Nurse for San Jose, CA (opportunity to earn $90k - $140k yearly) with multiple openings.

• We are offering a sign on bonus of $5000 for full-time RN and $2500 for part-time RNs
• Flexible Schedule - schedule your own visits
• Prohealth has 4 office locations servicing the Santa Clara, Contra Costa, Alameda, San Joaquin, and Sacramento area. Prohealth Home Care is seeking an empathetic Home Health Registered Nurse (RN) who will aim to make a positive difference in our community.     • Provide admission, evaluation and follow-up skilled nursing visits for home health patients
    • Provide quality hands-on care, management and teaching of patient's care plan in accordance with physician orders, under Clinical Manager's supervision
    • Coordinate appropriate care, involving all the other healthcare team members (such as Physical Therapists, Occupational Therapists, Home Health Aides and Medical Social Workers)
    • Report patient's progress to physician and Clinical Manager as required to maintain continuity of care
    • Implement plan of care with patient and family to assist them in achieving optimal outcomes POSITION REQUIREMENTS: Home Health Registered Nurse Qualifications:
      
      • Valid RN License to practice in CA
      • Current BLS/CPR certification
      • Valid CA Driver's license
      • Minimum of one year nursing experience is preferred
      • Med Surg, ICU, ER, Acute Experience, Home Health experience is a plus ***Wound Care experience preferred. ***
