Reports to : Chief Medical Officer/Medical Director Status :
Full-Time/Part-Time, Exempt Position Summary : Reporting to the
Chief Medical Officer, the nurse practitioner provides full-scope
family practice services under the guidance and supervision of
physician. The IHC is a Patient Centered Health Home and all
employees are an integral part of this model of care delivery.
Duties & Responsibilities : Provide direct primary care ambulatory
services to include examination, diagnosis, treatment, referral and
follow-up of patients. Providing service to pediatric, family
practice, prenatal, adult and general practice. Working in an
independent and interdependent relationship with members of the
medical staff The diagnosis and treatment of acute, chronic and
long-term health care Educating patients and/or family members
about preventive care, medical issues and use of prescribed medical
treatments and/or medications Provide all services in accordance
with accepted medical standards, organizational policies, and in a
manner that is responsive to the needs of all patients Maintain
complete, accurate, confidential and legible medical records
according to established protocols Participate in peer review and
meetings as assigned Participate as a proactive representative of
the Patient Centered Health Home Perform duties utilizing the
Team-Based Approach Perform other duties as assigned Required
Qualifications, Knowledge & Abilities : Nurse Practitioner: MSN
from an accredited nursing school --- specialty in Family Practice
preferred Three years--- work experience as a Nurse Practitioner in
primary care preferred National FNP licensure from the AANP or the
ANCC Current unrestricted California Nurse Practitioner license
Current DEA license and CPR certification Excellent customer
service skills Knowledge of and ability to relate to the American
Indian community and other minority populations Bilingual in
English and Spanish a plus but not required Flexibility and ability
to work as a team member Possession of a valid California Driver
License, automobile insurance, and a clean driving record Ability
to maintain strict confidentiality
