Job Description:
AbilityPath is hiring Case Managers for our Independent Living
Skills (ILS) Program!
Our program operates in Santa Cruz, Santa Clara and San Mateo
counties! Locations include: Santa Cruz, San Jose, Palo Alto,
Redwood City, & Daly City areas
ILS is a community-based program that specializes in teaching
adults with disabilities the fundamental skills needed to live
independently. The ILS case manager provides support in skill
development to prepare participants to move out on their own and
live independently, while also providing supports for participants
that are currently living alone but need assistance to maintain
their independence.
