PATH is seeking candidates excited about making a positive impact
in the lives of others to join our SHS Program under the ICMS
division as a Lead Case Manager. -
ABOUT OUR SUPPORTIVE HOUSING SERVICES Our Permanent Supportive
Services (PSS) programs provide case management services in a
Permanent Supportive Housing (PSH) setting to individuals who have
experienced homelessness and have a history of chronic health,
mental health, and/or co-occurring conditions. Services are
centralized and provided to clients living on-site to ensure ease
of access to the resources necessary for long-term housing
stabilization, improving their quality of life and community
integration.
WHAT IS A LEAD CASE MANAGER?The Lead Case Manager will work in a
permanent supportive housing (PSH) setting (low-barrier, high
tolerance) to provide case management services for tenants that may
have a variety of complex health, functional, and social needs. As
a Lead Case Manager V, this position will be required to serve as
the point of contact for specified funders, provide
support/training to new case management staff, facilitate trainings
on behalf of PATH, coordinate and train volunteers/interns, and/or
oversee specific administrative functions of the PATH PSH Program.
Responsibilities include:
Case Management Oversight -
- Support onboarding and provide guidance to new ICMS case
managers
- Oversee all phases of participant services including screening,
intake and enrollment, case management, and exit and follow-up
processes
- In collaboration with on-site Program Manager, assign and
adjust tasks and on-going assignments in accordance with
programmatic needs
- Co-lead staff meetings, case conferencing and the development
of action plans to resolve case-specific issues impeding client
progress, as needed
- Serve as site proxy for the disbursement of site and client
assistance funding as needed for the program.
- Provide direct services to a small group of participants as
necessary to support their needs and the needs of the program as a
whole
Program Administration
- Present regularly monitored data and program outcomes to the
Program Manager
- Ensure that all program data is accurate and entered the
required program documentation system as contractually
required
- Conduct employee performance management such as corrective
action planning for direct report staff in collaboration with
supervisor
- Supervise and work in collaboration with clients and case
managers on the development of individual housing plans to help
clients reach their goals
- Ensure proper documentation of site financial spending and
storage of reimbursement receipts according to department and
agency policies
- Participate and/or present at local meetings with community
partners
Documentation -
- Ensure, maintain and execute documentation standards as set
forth by the program contract and PATH polices, including
maintaining compliance with HIPAA policies and practices
- Complete and ensure the completion of progress notes and
service entry on every contact with clients or collateral contacts
by 5 pm the next business day
- Complete and ensure the completion of intake, assessment and
service plans in coordination with the client and within the
documentation standards set for the by the program contract and
PATH
WHAT YOU BRINGWe're looking for candidates that:
- Excellent leadership skills with a hands-on, lead-by-example
work style -
- Ability to maintain and execute confidential information
according to HIPAA standards
- A high level of tolerance and understanding for individuals who
present for services with urgent and multiple case management and
health needs
- Ability to work effectively independently and as part of a
team
- Mature, professional judgment and the ability to effectively
cope and resolve immediate conflict or crisis situations
PREFERRED QUALIFICATIONS -
- HMIS Experience
- Two (2) years of experience in healthcare, case management,
homeless services or working with vulnerable populations. -
MINIMUM QUALIFICATIONSFor this role a candidate must:
- Demonstrated knowledge of and experience with case management
techniques such as critical time intervention, harm reduction
strategies, crisis intervention techniques and motivational
interviewing
- One (1) years of experience in healthcare, case management,
homeless services or working with vulnerable populations and a
social work/mental health related BA. -
MANDATORY REQUIREMENTSRegardless of qualifications, candidates
must:
- Have employment eligibility verification
- Be able and willing to work flexible hours which may include
evenings or weekends
- Have or be able and willing to obtain CPR/First Aid
training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVINGDriving is an essential function of this position. To meet
the employment requirements for this role, all candidates must:

- A valid driver's license
- Proof of insurance and ownership for personal vehicles used
during work duties
